PREPARING FOR YOUR VISIT

Things to bring for your visit…


Thank you for choosing Pulmonary and Critical Care Medicine Associates. We want to make sure your visit is as smooth and convenient as possible.

To help us better serve you, please bring the following materials with you to your first appointment:

  • List of questions for your physician
  • List of all medications you are taking, including prescriptions and over-the-counter medicines
  • Referral from your primary care physician, if required by your insurance company
  • Any recent X-ray or CT scan of chest
  • Any recent tests related to your condition
  • Current insurance card, Photo ID, including a driver’s license or any other state or federal issued photo ID
  • List of prior surgeries and medical history

Please arrive 15 minutes prior to your scheduled appointment time in order to check in and register for your appointment.

Testing may be conducted on the day of your first visit. This may include a pulmonary function test (PFT), exercise desaturation test, and other tests as needed. Testing is performed in the office during your appointment, so please plan on being here for an extended amount of time for your first visit.
